The new Margaretta School-Based Health Center will provide tremendous benefits for our students, staff, and Margaretta families by increasing access to healthcare services right here within our village! From routine care and wellness support to unexpected illness, this center will play an important role in supporting the overall well-being of our Polar Bear community!

Located in the old Margaretta Board Office at 305 South Washington Street in Castalia.

This form must be completed annually for students to receive services or treatments. It is recommended that parents complete this at the beginning of the school year. 

Please call the Erie County Community Health Center Centralized Scheduling office at 567-867-5174 to pre-register your or your child's appointment. Walk-in appointments are available. 

Please bring a copy of the parent/guardian's photo ID (for minor children) and insurance card, if possible. It is not necessary for your to attend the appointment with your child if you are unable to.
